THE ORGANISATION
As Western Australia’s first Aboriginal foster care agency, Yorganop has been working in partnership with government agencies and our community of carers across WA for more than 34 years, to provide culturally appropriate care for Aboriginal children.
In 2022, Yorganop commenced the Home Stretch WA service which supports Aboriginal young people between 18 and 21 years old, transitioning from out-of-home care to independent living.
Our over-arching purpose is nurturing children to flourish, and empowering Aboriginal children, young people and families to grow strong in themselves, their culture and their future.

We are currently recruiting for a Home Stretch Coordinator, to join our amazing team which offers support services to young people who are leaving Out of Home Care the choice to access higher levels of support until they reach the age of 21. We work with young people 17 to 21 years of age to help ensure their transition to adulthood is as smooth as possible working with youth and supporting them to achieve their goals, connect with family, find stable accommodation, access employment and education opportunities, and connect with other support services
This role will assist with the implementation of effective processes, operating within the Home Stretch guidelines for the Home Stretch team to improve performance, productivity and outcomes. This will be done by communication, project management, decision making, problem solving and collaboration. This role will Supervise and mentor Transition Coaches and the Staying On Facilitator.

ABOUT THE ROLE
Essential criteria:
Demonstrated knowledge and understanding of Aboriginal culture and the ability to work with Aboriginal young people, communities, and families.
Demonstrated experience in engaging and building strong relationships with and motivating ‘at risk’ Aboriginal young people.
A thorough understanding of the barriers and difficulties facing Aboriginal young people with experiences of out of home care.
Ability to be innovative and flexible, work independently, demonstrate initiative and work effectively under pressure.
Ability to assist Aboriginal young people to develop independent living skills.
Strong written and verbal communication skills.
Organisational skills to prioritise tasks and manage time effectively.
Demonstrated behaviour management, conflict resolution, advocacy and negotiation skills.
Willingness to work across the local region on regular outreach: working in the client’s home, in local community centres and any other venue where a young people may require support.
Ability to work in a team and to work collaboratively with others.
Desirable criteria:
Good understanding of drug and alcohol and mental health issues and support services.
QUALIFICATIONS AND LICENSES
Essential:
Qualification/s: Bachelor of Diploma in a relevant field is preferred however unqualified applicants with a minimum of 2 years demonstrated relevant experience and skills working with young people with experiences of Out-of-Home Care will be considered.
Criminal Record Screening Appointment: is subject to a satisfactory National Police Clearance that must be maintained for the duration of your employment and a Department of Communities Departmental Check (Departmental Record Check & Criminal Record Check) that must be maintained for the duration of your employment.
Appointment is subject to a satisfactory: Working with Children Check that must be maintained for the duration of your employment.
