About the role

Disability Support Link is seeking experienced Social Workers/Support Coordinators to join our growing team across Melbourne and Regional Victoria (Geelong, Ballarat and Bendigo). This is a Work from Home contract position that plays a vital role in our mission to empower individuals with disabilities and support needs. As a Social Worker/Support Coordinator, you will be at the forefront of delivering person-centered support coordination services, helping clients navigate complex systems, access resources, and achieve their personal goals. This role is fundamental to our organisation's commitment to improving quality of life and promoting independence for the vulnerable populations we serve.

Key responsibilities

  • Conduct comprehensive assessments of Participant's needs, circumstances, and goals to develop individualised support plans

  • Coordinate and facilitate access to community services, health care, housing, employment, and other relevant support services

  • Provide proactive support, counselling, and advocacy to participants and their families

  • Develop and maintain strong relationships with service providers, government agencies, and community organisations to ensure seamless service coordination

  • Monitor and review progress against support plan objectives, making adjustments as required

  • Maintain detailed and accurate case notes and documentation in compliance with relevant legislation and organisational policies

  • Prepare reports and recommendations for case conferences, tribunal hearings, and other formal proceedings

  • Provide crisis intervention and emergency support when required

  • Provide inclusive and ethical service respecting Participant's Choice and Control

  • Participate in professional development activities and contribute to a culture of continuous improvement


What we're looking for

  • Tertiary qualification in Social Work, Community Services, Mental Health, Disability Support, or related field

  • Current professional registration or accreditation with relevant regulatory bodies where applicable

  • Minimum 1-2 years' experience in aged care, disability support, community services, or related social services environment

  • Strong knowledge of the National Disability Insurance Scheme (NDIS), relevant rules and legislation

  • Demonstrated ability to work with diverse client groups including people with disabilities, older adults, and vulnerable populations

  • Excellent communication, interpersonal, and active listening skills

  • Strong work ethics and time management abilities with capacity to manage multiple cases simultaneously

  • Proficiency in case management systems and Microsoft Office applications

  • Current understanding of safeguarding, privacy, and confidentiality principles

  • A compassionate, non-judgemental approach with genuine commitment to client empowerment and independence

  • Ability to work both independently and collaboratively within a team environment

  • Valid Driver's License, own car, home office set up and ABN
